Chest Pain (Low Risk) HPI

General Chest Pain
Pain described as gradual and episodic, worsening of chronic pain.
Patient is not diaphoretic during episodes and it does not radiate away from epigastric area or central chest. Patient describes history of similar pain. Denies exertional component.
No known h/o Hypercholesterolemia, Hypertension, Diabetes Mellitus, Smoking, Obesity. Pt denies personal history of cardiac disease and family history of cardiac disease under the age of 50yo.
Patient denies SOB. Pt without dysuria or changes to urinary/bowel habits. Denies F/N/V.

Anxiety Chest Pain

Patient with history of anxiety describes acute chest pain episode that is nonexertional in nature.
Patient is not diaphoretic during episodes and it does not radiate away from epigastric area or central chest. Patient describes history of similar pain.
No known h/o Hypercholesterolemia, Hypertension, Diabetes Mellitus, Smoking, Obesity. Pt denies personal history of cardiac disease and family history of cardiac disease under the age of 50yo.
Pt denies any ingestions and denies any other medical complaints. Denies AH, VH, SI, HI. Also denies any prior suicide attempts.
Pt states they have felt otherwise in their normal state of health with no fever, N/V, chills, Neck pain, HA, new cold/heat intolerance, dysphagia.
