HPI Templates

Hematuria

Denies dysuria or recent UTI.
Denies blunt trauma to abdomen or genitals. No urethral instrumentation.
No colicky flank pain and no history of kidney stones.
No other bleeding sources or bruising. No NSAID use or sickle cell disease.
No receptive rectal sex or rectal instrumentation.
Nonsmoker. No cancer history.

Urinary Tract Infection

No history of UTI previous to this visit within one month or one year. Patient not taking any immunosuppressive medications and does not have known history of AIDS or cancer.
Denies h/o STDs, genitourinary discharge. Patient has not recognized any genital skin lesions.

Denies Abdominal Pain.
Denies nausea, vomiting, and diarrhea. Denies h/o abdominal surgery, kidney stones. No recent binge drinking episodes.

Denies Back Pain.
Denies sudden onset or otherwise unexplained lower extremity weakness. Denies Saddle numbness, hypoesthesia, or anesthesia.

Patient has otherwise been in their normal state of health.
Patient currently tolerates regular diet.

Vaginal Bleeding

Denies changes in hair distribution, no weight change, no change in acne, no visual changes, no sensation of inappropriate warmth or cold.
No oral or other mucosal bleeding identified.
Denies history of vaginal/uterine instrumentation, hard sex, or sexual abuse.
Patient is monogamous with sexual partner also believed to be monogamous, no h/o STDs
Otherwise in her normal state of health and denies any F/N/V/D.
