#1 Neurologic Exam:
General: Patient is alert, attentive, and oriented. Speech is clear and fluent. PERRL and there is no facial droop. There is no pronator drift of out-stretched arms.
Motor: Muscle bulk and tone are normal. Strength is full bilaterally in UEs and LEs
Sensation: SILT in all four extremities.
Coordination: No dysmetria on finger-to-nose and heel-knee-shin. Gait is steady with normal steps.
#2 Neurologic Exam:
General: alert and oriented, mood and affect normal
Speech: normal, no aphasia or dysarthria
Cranial nerves: CN2-12 in tact
Peripheral exam: motor and sensation normal, reflexes normal, no pronator drift, normal finger to nose
Gait: normal with normal Romberg and tandem gait
