Nerve Block Procedure

Radial/Median/Ulnar
The patient’s ___ upper extremity was addressed.
A time out was undertaken to determine that this was the correct patient and the correct procedure for this patient.
The volar distal forearm was prepped and cleansed in the usual sterile fashion.
Bupivacaine 0.5% plain was injected around the nerve bundle under ultrasound guidance.
A total of ___ mL were used to infiltrate around the ulnar nerve.
This provided patient with excellent analgesia. The patient tolerated this procedure well there were no complications.

Fascia Iliaca Block
The patient’s left lower extremity was addressed.
A time out was undertaken to determine that this was the correct patient and the correct procedure for this patient.
The hip/groin was prepped and cleansed in the usual sterile fashion.
Sterile US guidance was used. The needle was visualized passing through the fascia lata and fascia iliaca.
Marcaine 0.5% was injected with visualized hydrodissection below the fascia iliaca lateral to the femoral nerve.
A total of ___10 mL was used to infiltrate around the nerve.
This provided patient with excellent analgesia. The patient tolerated this procedure well there were no complications.
