CVC Procedure

CVC ACCESS PROCEDURE NOTE INDICATION:
Emergency vascular access for fluid and drug administration.
INFORMED CONSENT: The risks of the procedure including bleeding, pneumothorax, and infection were explained to the patient. The patient verbalized their understanding of the procedure, the risks, benefits and alternatives and wished to proceed.

PROCEDURE:
The patient was placed in Trendelenburg and the ___ area was prepped and draped in a sterile fashion.
The area of interest was anesthetized with local anesthetic.
With the introducer needle bevel oriented inferomedially, the ___ area was entered shallowly advancing with continuous aspiration on the syringe until the ___ vein was entered and there was free flow of venous blood.
A Seldinger technique was then utilized to place a central venous catheter over a guidewire and the catheter was secured in place.

A Portable Chest X-ray was ordered to confirm the catheter’s position.
EBL: < 10 mL
COMPLICATIONS: None
